LuxSmile Dental Ltd. is a professional dental hygiene practice operated by Leili Zarrabi, Registered Dental Hygienist (RDH), based in Toronto, Ontario, Canada. All dental hygiene services are performed in accordance with the standards set by the College of Dental Hygienists of Ontario (CDHO).
LuxSmile provides dental hygiene services including, but not limited to, teeth whitening, routine dental cleanings, scaling and root planing, fluoride therapy, oral hygiene instruction, and mobile dental hygiene. All services are subject to a clinical assessment and may be modified or declined at the professional discretion of the hygienist.
Payment is due at the time of service. We accept major credit cards, debit, and e-transfer. LuxSmile is not responsible for direct billing to insurance providers; however, we are happy to provide receipts for your insurance claims.
Clients are required to disclose accurate and complete medical and dental history prior to treatment. LuxSmile is not liable for adverse outcomes arising from incomplete or inaccurate health information provided by the client.
LuxSmile Dental Ltd. and Leili Zarrabi, RDH, shall not be held liable for any indirect, incidental, or consequential damages arising from the use of our services or website. Our liability is limited to the cost of the service provided.
